Preschool Registration
MILFORD PRESCHOOL REGISTRATION FOR 2023-24 SCHOOL YEAR
PRESCHOOL APPOINTMENTS CAN BE MADE STARTING TUESDAY, JANUARY 3RD.
Due to limited availability, Preschool /Extended Day services are on a first-come, first-serve basis. Making an appointment to register and completing FinalForms does not guarantee spots will be available.
Should classes be full, we will offer to put you on a waitlist for the program. We cannot hold spots if any of the required documentation is incorrect or if FinalForms is not complete at the time of your registration appointment. Should this occur, you will need to make another appointment to register your child.
Milford Preschool will be accepting new student registration applications by appointment for the upcoming school year on the following dates/times:
Tuesday, January 31, 4:00 pm – 7:00 pm
Wednesday, February 8, 4:00 pm - 7:00 pm
Friday, February 10, 9:30 am – 3:00 pm
Please call the Milford Preschool/Extended day office at 513-728-7455 to make a registration appointment. Registration appointments will be in 10-minute increments. Should you arrive early, please wait in your car until your appointment.
AFTER MAKING A REGISTRATION APPOINTMENT -
First step: Complete the online Intent to Register.
Second step: To access FinalForms, click on the FinalForms Link. You will start by creating a parent account. You will then sign into that account and select ADD NEW STUDENT to create your student’s account. The forms will populate in a list shown in red. Click on each tab, answer all questions completely, and "sign" each form by scrolling to the bottom of the form, typing your name, and clicking the submit button. Each form will turn green when completed.
Third step: Scan/take a photo of the following documents and upload them directly into the student’s FinalForms account. (Please note that you are required to provide all of the documents listed below to register for Milford Preschool).

Color scan of the original birth certificate

Color scan of parent driver's license

Proof of Residency (must be a MEVSD resident to attend) – We can ONLY accept the following as proof of residency: utility bill (electric/water), lease/rental agreement, or If you live with a Milford resident and cannot provide one of the above, you may be considered for a residency affidavit. Contact the Milford Administrative Offices at 513.831.1314 for further details.

Court/Custody Documents (when applicable), i.e., Shared Parenting, Divorce Decree, etc. (full document, no redactions; filed and stamped).
FinalForms must be completed, and all required documentation must be downloaded on FinalForms before your registration appointment.
To complete the registration application, you must bring a $65.00 registration fee (payable by check or money order payable to Milford Preschool) to your registration appointment. We cannot accept debit/credit cards for the registration fee.
PRESCHOOL/EXTENDED DAY TUITION FOR THE 2023-2024 SCHOOL YEAR
PRESCHOOL ONLY
3-yr old Typical Preschool*(Mon-Thu, 9:05-11:35a) = $59.00/weekly
4-yr old Typical Preschool* (Mon-Thu, 1:05p-3:35p) = $59.00/weekly
3&4-yr old Special Needs Placement No Preschool tuition as required by law
PRESCHOOL WITH EXTENDED DAY
4 days Extended Day w/Preschool (Mon-Thu, 6:30a-6:30p) = $191.00/weekly (Typical), $132.00/weekly (S/N)
5 days Extended Day w/Preschool* (Mon-Fri, 6:30a-6:30p) = $204.00/weekly (Typical), $145.00/weekly (S/N)
*Preschool children are in Extended Day all day on Fridays.
REMEMBER THAT YOUR CHILD WILL NEED TO BE TOILET TRAINED (SPECIAL NEEDS EXCEPTION), HAVE AN UPDATED MEDICAL AND DENTAL EXAM, AS WELL AS PROOF OF IMMUNIZATION IN ORDER TO ATTEND.
TRANSPORTATION IS NOT PROVIDED.
AFFIDAVITS OF RESIDENCY
The Milford Exempted Village School District Residency Affidavit forms must be completed if the custodial parent/guardian is living with a Milford Resident and does not have a lease or mortgage in their name.
The residency affidavits must be signed and notarized in the presence of a Milford staff notary. You must call 513-576-4178 to make an appointment for the notary. Owners/Residents must also provide proof of residency at the time of notarization (utility bill issued within 60 days, cannot be a disconnect notice), or lease agreement.
Please contact the Central Registration Office at 513-576-4178 to obtain specific information regarding what documents are necessary to provide proof of residency in addition to completing the attached forms.
