HealthSource Mobile Vision Coming to Milford Schools November 2–10
Good vision plays an important role in student success, and Milford Schools is partnering with HealthSource of Ohio to make vision care more accessible for our families!
From November 2–10, 2026, the HealthSource Mobile Vision team will visit Milford Schools to provide comprehensive eye exams for students across all grade levels. These services will be available directly at our school buildings, helping families access important vision care without the need for a separate trip to a doctor's office.
According to the American Optometric Association, approximately one in four school-age children has a vision disorder. Because children may not recognize or communicate vision difficulties, regular eye exams can help identify concerns that could affect their learning.
Through HealthSource's partnership with the Ohio Department of Health, eligible students in kindergarten through third grade can receive FREE vision services, including comprehensive eye exams and eyeglasses if needed. Students in other grade levels may also receive services, and most insurance plans are accepted.
How to Register Your Student
Families interested in having their child receive vision services must complete a two-step registration process:
Submit the initial request form. Families should use their child's full name and date of birth when completing the form.
Complete the consent and health history forms. After submitting the initial request, families will receive an email by the next business day with a link and login information to complete the additional paperwork.
Both steps must be completed for students to receive services. Families should ensure their child's name and date of birth match across all forms.
Once completed, the consent remains valid while the student attends their current school district. Health history information must be updated annually for continued services.
