Milford Preschool

Milford Preschool/Extended Day Registration 2020-2021

Milford Preschool registration nights will be held on Wednesday, February 5th from 4-7 pm and Thursday, February 13th from 4-7 pm.  Milford Preschool registration is open to Milford residents only.  If you reside outside the district, you may apply for open enrollment beginning in April.  In order to register your child for Milford Preschool, you must bring the completed registration packet, all necessary documentation and the $65.00 registration fee the night of registration.  The registration fee must be paid by check or money order - we cannot accept credit/debit cards for registration fees.  Please be sure to read the cover page of the registration packet completely.  The cover page contains all information regarding rates, hours, documentation needed, etc.  We cannot hold spots if any of the required documentation/fees are incomplete or incorrect on the night of registration.  Please call 728-7400 if you have any questions.

**IMPORTANT**   A/O 1/23/20 - the 4 day Extended Day rate was incorrectly posted on the registration cover page.  The rates for 4 days of preschool with 4 days of extended day are $191.00/week ($132.00/week for special needs).  The corrected Preschool Registration Packet is below:

Please click on the link below to download and print the Required Forms packet for the 20-21 school year.  Per ODE all children must have these forms on file in order to attend the preschool.  Doctors/Dentists can fax their completed forms to our office at 831-8764.

AFFIDAVITS OF RESIDENCY 

The Milford Exempted Village School District Residency Affidavit forms must be completed if the custodial parent/guardian is living with a Milford Resident and does not have a lease or mortgage in their name. 

The residency affidavits must be signed and notarized in the presence of a Milford staff notary. You must call 513-576-4178 to make an appointment for the notary.  Owners/Residents must also provide proof of residency at the time of notarization (utility bill issued within 60 days, cannot be a disconnect notice), or lease agreement. 

Please contact the Central Registration Office at 513-576-4178 to obtain specific information regarding what documents are necessary to provide proof of residency in addition to completing the attached forms.
